ABSTRACT

Health insurance reform, which was originally scheduled in 2000, was cancelled. One of the main reasons is that the debate has been limited on the issue, "Which is better to get the fund of healthcare for over 70 elderlies, Social Premium or Tax ?", without making a consensus on the cost sharing rule between elderlies and active workers. The beneficiary cost sharing ratio of medical and long-term care cost for elderlies is currently estimated at 24%. At first, we have to make the consensus on how much this ratio should be raised in the process of aging.

The first proposal is to keep the increase ratio of cost sharing per capita at the same between elderlies and active workers, on condition of the national medical and long-term care expenditure, including not only the cost for elderlies but also the cost for non-elderlies. However, in this case, the cost sharing per capita will become at least 1.4 times on real term in 2040 and the beneficiary cost sharing ratio mentioned above will rise from 24% to 34%. Therefore, we should curtail the healthcare expenditure and mitigate the cost burden on people.

Since the increase of medical cost is unavoidable in the process of aging, it is more suitable as a goal of health insurance reform to establish the mechanism, with which the system can be stable under the cost increase. The second proposal is to introduce the health insurance plan for elderlies with two-tiers of basic benefit and supplemental benefit.

To eliminate the waste of money in healthcare and raise the satisfaction of people, it is necessary to create the device under which the participants in the regional area unit for healthcare own jointly information and management resources. The third proposal is to establish Integrated Healthcare Delivery Network.
